If you have an ASIC or MSIC, you may need to request a replacement card if:
- you have changed your name
- your card has been lost or destroyed
- your card has been stolen
- your card is damaged or faulty.
Change of name
If you have changed your name, you must provide AusCheck, in person, with a document issued by a Commonwealth, state or territory department or agency that shows the change. This may include a change of name certificate issued by Births, Deaths and Marriages in your state. You must also provide AusCheck with a statutory declaration. This information can be included in the same statutory declaration referred to below in Additional requirements.
Lost or destroyed card
If your card has been lost or destroyed, you must provide AusCheck with a statutory declaration outlining the circumstances of the loss or destruction. This information can be included in the same statutory declaration referred to below in Additional requirements.
Stolen card
If your card has been stolen, you must provide AusCheck with proof that the theft was reported to police within 7 days of becoming aware it was stolen. This may be a copy of the police report, a police report number or other information issued by the police. This information can be included in the same statutory declaration referred to below in Additional requirements.
Damaged or faulty card
For damaged or faulty cards, you must return the card to AusCheck when you are issued with your replacement card.
Additional requirement(s)
If the card is being replaced because of a change of name, loss, destruction or theft, you must provide AusCheck with a statutory declaration. The declaration must outline any relevant changes since your background check was completed. It must also confirm that you have not been convicted of an aviation security-relevant offence (if you are replacing an ASIC), or a maritime security-relevant offence, (if you are replacing an MSIC).
If you have not been convicted of any aviation or maritime security-relevant offences since your background check was completed, you must use this exact wording in your statutory declaration:
- For ASIC: Since my background check was completed, I have not been convicted of an aviation security-relevant offence.
- For MSIC: Since my background check was completed, I have not been convicted of a maritime security-relevant offence.
You must report any new convictions for any security-relevant offences to AusCheck or your issuing body within 7 business days of the conviction.

Request your replacement card
Once you have all your required documents, for example a statutory declaration, follow these steps to request a replacement card.
- Step 1: Go to the applicant portal (Opens in a new tab/window)(Opens in a new tab/window). You'll be redirected to sign in via the myID app.
- Step 2: From the left-hand menu, select Security Cards.
- Step 3: Select your ASIC or MSIC from the list and select Action.
- Step 4: Upload your statutory declaration. If your card was stolen, and if applicable, also upload your police report.
Your employer must approve your operational need for the replacement card in the employer portal. Let your employer know you've submitted the replacement request and it is ready for their approval.
